COMMUNITY HOME CARE AGEING AT HOME
We are a not-for-profit community organisation that provides local care coordination of Support at Home funding to residents in Loch Sport and surrounding areas.
About Us
Your home care journey starts here.
To assist senior Australians with living independently in their own homes, the Australian Government has created the Support at Home (SaH) Program.
Community House Care is a not-for-profit community organisation that provides local fully coordinated support to manage your SAH in the remote communities of Loch Sport and surrounding areas.
We aim to employ locals and use local businesses to provide the support for our community whether they are living in town or on one of the many surrounding rural properties.
As care coordinators, we have partnered with Trilogy Care as the Support at Home funding provider. Community House Care is built by a team of locals dedicated in providing easily-accessible home care supports.
Why Community House Care?
Remain healthy, independent, and connected.
Community House Care will work alongside you to manage tasks that many find difficult, such as finding appropriate support workers, scheduling care hours, and managing your budget.
We work with you and the local workforce to meet your care needs. Once you have selected your preferred support worker, we run compliance checks to ensure all workers have a registered ABN, current insurance, and any licenses required to carry out their specific line of work.
Working with your local Community House Care coordinator – that is powered by Trilogy Care, an Australian-owned aged care provider, means you can be assured you are receiving the best quality care, offering you more flexibility and value for money.

What is an ACAT Assessment
The Aged Care Assessment Team (ACAT) will assess your health, lifestyle and living situation, from this information they grant packages to consumers on four different levels of care:
SaH Level 1: $2,752 per quarter
SaH Level 2: $4,112 per quarter
Sah Level 3: $5,634 per quarter
SaH Level 4: $7,617per quarter
SaH Level 5: $10,182 per quarter

What services are available with a Support at Home Package
There are various services you are able to receive on a Support at Home funding (SAH). SAH funds can be used for home care services which assist you in remaining healthy, safe, independent, and connected to the community. Services that are specifically excluded by the Australian Government from being covered with SAH funds are excluded because they do not meet the immediate care needs of consumers.
